This was my first experiment, the measurements were far from exact, but I was going for about 14mg nicotine.

Mixed the following:
Approx 2 parts 36mg liquid
Approx 2 parts Glycerine
0.5 part chocolate flavour
0.5 part rum flavour.

Results:
Delicious! But the flavour was a little overbearing, so I added unknown quantities of Glycerine and e-liquid until it tasted perfect. However, I think I put too much e-liquid in, so the final product was probably more like 16mg nicotine. Excellent throat kick. Nice balance between the chocolate and rum flavours. It tasted exactly how I had hoped.
