When they quit thinking a PV is an electric cigarette, they'll quit treating us like smokers and calling vapor smoke. Best not to vape what looks like a cig. or use the word e-cigarette among the uninitiated. Vaporizers are "e-cigarettes" like electric toothbrushes are e-carrots.

I'd forget about the 18350 and go with an Icon or Mako Midi, both of which are fully mechanical 14500 mods. If you want more battery than a 14500, get a Mako Maksi which uses a 14650 bat (the Maksi tube is only 10mm longer than the Midi)

The Don is a good choice if you still want to use the 18350, but also have the ability to use a 18650. The Don and Mako are made by the same company.

This mod gripped my attention.
The pocket-size ecigs I have are not pocket-size enough. The smallest one I have is an Ego 650mah. Attaching a carto and a drip tip adds to the length, which is not exactly safe and comfortable in the pocket. So it's not really the width or fatness that's causing the problem. It's the unnecessary length. I also have a 2AA box mod and have been trying to find time to modify it and come up with one that puts the connector all the way inside the box.
Anyways, such config like the Gripper in the photo is the only design you can come up with to solve the length issue.
I might have one of these Grippers someday. I've read some issues about them like connector inaccessibility, etc. But it's definitely something that manufacturers should consider adding to their future products.
One obvious modification would be to just add some kind of a clamp, instead of a continuous tube housing the carto, which would expose the connector for accessibility.
I laud design efforts such as this, nonetheless.
Another option is the 18350 cube by siam mods which I think is a beauty. They also do amazing glass tanks.
I think you should check out the Puresmoker Legacy mini PureSmoker LEGACY Mini [AW IMR 18350]. Takes an 18350 and is quite small. The cube is nice, but I've never seen one available. Here are some pics of the mini next to some other things for reference:

Reasonable price too. Oh, and another thing cool, they sell an extension sleeve, screw that on and it can take an 18350 + a Kick.
GLV super mini is also a great choice, smaller and doesn't take 18350. Shipping time is really long though.
