But don't you need some kind of document certifying your medical needs?
I know that TSA routinely confiscates sharpies by default.

Well technically syringes need to be accompanied by medication.
But in reality the only thing on a TSA agents mind is when their next "smoke" break is
. And besides if they do throw it away, you lost a 25 cent disposable syringe, they are not going to arrest you.

OK, I'd agree with that! But I think the chances of a syringe going thru by mere luck are not high. And if I don't have 1 in my checked-in luggage, then I don't have my preferred method of refilling while I'm away.
Next time I'll put a syringe in BOTH check-in and carry-on, just to see what happens
