that "2" on the logo looks kind of familiar but I just been searching for about 15 minutes and can't identify it

Well considering it appears the atty, dt, and mod are all in-tact i find it highly unlikely that the "mouthpiece went down his throat and exploded again".

My bet is he was hitting it when it vented and he inhaled/took a direct hit from the outgassing/results of the thermal runaway.

This is a sad reminder to those who like to post threads saying "Stop saying batteries are dangerous, they're not", in the wrong hands/wrong situations, they can be incredibly dangerous.

That would appear to be a mechanical mod. If I was running a vape shop I wouldn't sell them. There is a little known refinement to Ohm's law. Ohm's are inversely proportional to masculinity. Mechanical box mods are a bit safer because they can vent more reliably. Tight fitting tube battery cases are just a bad idea.
