I find if I use a Cherry Ming drip tip I don't have any issues with that as it fits nice and snug, mostly have the issue with the generic drip tip that comes with the tank as it slips in the carto easily when turning.

I use the smoketech pre punched cartos, I haven't had a problem with those unless the carto was bad from the start. They also have one small hole closer to the bottom, using 50/50 here and not having any problems with it. My guess is the hole is located a little to high.
