Mostly the ability to use a larger range of resistances in atomizers / cartomizers. Past that, the ability to fine-tune your vape, as in finding the voltage you like to use most with the cartos you pick up.
You can use a tank on your KGO if you want, and they make 5ml cartos that should work...
I would think that the less they're used the more bacteria buildup there will be. If you're cycling juice through them on a regular basis there shouldn't be too much of that. When it starts to taste bad or gets off-color and gross, throw it out.
I would say when the taste becomes unacceptable to you or it stops working. I haven't run one to the dead point yet, but I've only been refilling blanks for about 5 days and haven't seen any problems with mine yet.
Can't hurt trying some pure VG juice. I used to have the dry eye issue with combustibles, haven't had a problem since switching over. Everybody's body reacts to things differently.
