A cartomizer has filler material inside the tube. You saturate the filler (lowest juice capacity is right around 1ml) and it wicks to the coil as you vaporize the juice.

I sub to a bunch of reviewers, but I'm usually pretty quick to jump on and watch videos posted by these guys.
One of my friends has mentioned that piece before, but I have never received a carto that came with the second piece. He just discards them and uses a drip tip, but if you prefer the vape with it in then leave it in. Just seems like it would be a pain to remove every time you refill.
You'll waste some juice on the initial fill, but as long as you're refilling regularly you should get good life out of the carto. As far as juice in the mouth, put a drip tip in there! Should eliminate most of that issue.
I'm not familiar with a second piece of rubber. Basically before you fill or put a drip tip on, you'll want to remove the little rubber doughnut from the open end of the carto. If there is a second one in that end, take it out as well. This is assuming we're talking about normal cartomizers...
Seriously, I can't get this thing to pop out with just my thumb. I'm sure I can grab it from both sides and pull it out, but thumb at the base with quite a bit of pressure out and up hasn't unseated the thing.
