If its the new starter pack pcc the clearances are pretty tight. You might be able to tap out one of the slots but it would be tricky. The cartos on the first page of this thread from vapor kings fit the pcc fine and the slots.

Sounds like a charger glitch to me. Happens all the time with cheaper batt types ie drills screwdrivers other cheaper rechargeable stuff. Seems feasible that it could happen with some of this gear.
