I'm still using some ancient cartomizers that only hold 0.8ml, but the newer ones all seem to hold 1, maybe 1.1ml.

I shake mine only to mix flavors. Has to sit for quite a while to allow all the air bubbles to escape, especially since I keep the juice refrigerated. Never noticed any separation of constituent parts.

I have a liter bottle of juice that I keep wrapped in brown paper (to protect from light) in the refrigerator. Should some judge or legislator make a stupid move, I'll be set for long enough to move to a more tolerant country. ;) The cartomizers I just leave at room temperature in ammunition trays.
