Heat is the enemy of batteries. Depending on how hot that battery got, and how many times, it might be toast, even if it was okay to start with. Make sure you get authentic, good batteries. Let us know how it works out, good luck.

Thermal runaway is when your battery gets so hot that it vents, which can be quite violent. Basically it's one step short of 'explosion'. I have no way of knowing whether it's the mod or the battery, but I would guess the battery. There are a bunch of fakes out there.

stop using it immediately. if it's that hot, it's not far from thermal runaway. you might have gotten a fake battery, or something. especially in a regulated mod, your batteries should never get too hot to handle.
