So I was working on the sheet like you said and I had the box open to add the page for everyone to use, I accidentally closed the box and now the option under tools is gone.

kwalka, if you go under the Tools menu, select 'Go to Live Form'. This will open the form, copy the URL from the address box at the top of the browser and post that for people to go to the form. (should work like that, I think )

I am able to adjust the access with the sharing like you said before. I was trying to add the form so they could just add there name and it would auto add them to the sheet, without them being able to edit the sheet. The option to do this was under tools like you said, but when I accidentally closed the secondary box, the option disappeared.

I think you did add a form to the spreadsheet. Actually, it's the 'Form(0)' menu you need to go to and then 'Go to Live Form'.
