Do I need rolygate's permission to reformat and reprint "The Great Nicotine Myth"?

A new B&M has opened near me, The Village Vapor Company (West Palm Beach, FL). Owner is a member of ECF("ennagizer") and active in CASAA.

He has a selection of reference material available for his customers; I've recommended this article and he agrees it would be a good addition.

Reformatting would involve only format changes to the text (to fit it into fewer pages and eliminate webpage sidebars). The final copy would include the url and copyright notice.
