E-juice tracker

Anybody have an e-juice spreadsheet or Word template they use to track the e-juices they've sampled? Okay, maybe I'm too OCD or a-retentive, but I'd like to be able to document the juices i've tried and the different characteristics of the vape so I don't forget and reorder something I don't like. Things I probably want to note are:

Manufacturer
E-juice name
PG/VG ratio
nic level
atomizer used
atomizer resistance
battery/mod used
voltage used (for vv mods)

Anything else?

Having re-read that list I realize I am definitely verging on OCD.

I'm looking for a spreadsheet, too. Jostumidd, you must have one put together by now, don't you?? It's not OCD - I'm thinking we also need to track:
Born on Date/purchase date
Personal Rating/Tasting Notes
Steeping date info

I'm sure there's more

i have a great xls it has about 80 flavors now, date ordered but not arrived or steeped dates. to compensate i only review after 14 days minimum, follow up after 30+ days if i notice changes. no ohms because all 2.4 ohm and 3.5-3.8v and Vision 2.0 clearos
