I apologize, as I'm sure this has been asked numerous times before...

I'm a fairly heavy vaper, probably vape about 4-5ml's a day on average. How long could I expect a fresh cartomizer to last?

I vape constantly -- usually take a couple drags every 5-10 minutes. One carto gets me thru the work day, and I usually start a new one in the evening. So 1.5-2/day is my average. I used to smoke 2 packs/day.

Edited to add: per other posts I've read here, each carto from V4L has about 0.8ml of juice in them. So you might use more than I do.

New carts are less than 1ml, (although Steve is working on this) When refilling or filling blanks you can get about 30 drops in a cart by shaking the cart or using the spin method.
