Oh no! My guess is that you used a citrus-type or cinnamon juice. These juices (and others) will quickly crack, break, sometimes even melt polycarbonate tanks. With the wrong juice, your clearomizer can be destroyed in a matter of minutes. Unfortunately the iClear16s are made out of polycarbonate.
The same thing happened to me when I got my iTaste MVP. My first iClear broke around the plastic where the drip tip connects, and the other around the base like you described. I only recently discovered what they problem was, and only after destroying two Kanger EVODS as well! hehe
