Muddled!

Math is not my strong suit. I have bottles of PG/VG mix at 15mg nicotine, and at 30mg nicotine. I want to vape at 18mg nicotine. I've decided on the mix...

22ml of 15mg nicotine
+8ml of 30mg nicotine
=30ml of 19mg nicotine
Mix will be diluted by flavouring so it should be close enough.

Am I wrong with the measurements?

Okay, great!

So, if you add 20% flavor, that will be 4 ml of your nic base + 1 ml of flavoring (for more than 5 ml, the proportions would still be the same, 5 ml is just an example). That would give you 76 mg of nicotine divided by 5 ml total, giving you a final nic concentration of 15.2 mg/ml. If you add 10% flavoring you would have 85.5 mg nic/5ml = 17.1 mg/ml nic.

Then your PG will be slightly more than 80% and VG slightly less than 20% (assuming your flavors are in PG), but shouldn't be too far off.
