Notifications sometimes not working?

In a contest thread I was tagged (@fatum) but I didn't get a notification. Then I was tagged a second time, alas, still no notification. Is there a setting that I'm missing or is this a forum bug?
I have everything under Alert Preferences set to on.

I should also add that if the post is edited and the "tag" is added after the fact, an alert is not sent.
