Is it true that this e-cig keyring thats the size of a medium cartomizer actually has a higher underload voltage than a fully charged eGo battery? How is that possible if it is in fact true? Does anyone know about this?

Its probably true because the Joytech egos which are the originals are regulated down to somewhere around 3.3 volts. Other "off" brands are generally unregulated which means that they would come off the charger at 4.2 and then slowly dwindle down much like any unregulated li-on batteries
