I noticed that after using a cartridge for a while, the tip tends to come off. Do people use the carts like this? Is there some kind of mod that solves this problem?

What like the little piece that seals the end where your mouth goes?
If thats the case just toss it, worst that can happen is you get a little liquid in your mouth.

I just pitch them. I think it's from biting the whistle tip. Just be glad you didn't inhale it.

The first time that happened I had a serious WTF! moment as I spit out some weird brown object until I realized what it was. I was putting them back in but found that once they come out they continue to do so and I just toss that piece.
