You have:
+/- battery in
+ 510 out to center pin
- for the chip
- for the 510 adapter

I can't really make a suggestion as I don't know what 'out' is. Unless your saying you put the + out to center pin to a ground? The metal plate sounds like it's complicating things.

In the first picture its opposite to what it says in the diagram and on the board itself but thats how it was hooked up. The ground was on the metal holder and the out was connected to thw 510 pin.

Why don't you post the diagram and version of your Tmax chip?

im not sure what version i have. also heres a picture close up of what the board says.
the gnd was hooked up to the bottom of the 510 and the out was what was grounded on the metal holder.
honestly ill hook it up the way it says on the board.

I wouldn't doubt the PCB labels.

Your LCD screen seems skewed a bit?

yea a little. it was like that when i took it out. from the videos and things ive read its a pretty common problem.
