Madvapes makes a 5 volt and vv model of a small box mod. They are about 30 bucks. Very cheaply made but for the few months that they actually work, they're pretty awesome. About as small as ive ever seen.

Also, the word 'mod' is loosely defined. I generally look at anythin other than a cig-a-like or ego style as a mod these days.

As some folks mentioned before, the smallest mechs out there are any that can run on an 18350 battery or a 14500 battery. There is the Microstick MicroStick (shipping in 15-30 days after payment) - Mini ECIG looks super sweet, but you do need an atomizer that can fit into the chamber. It's 100% mechanical as well. There are some pretty tiny eGo's out there too. Just saw a 400mAh Spinner. That's always an option if you want something really stealthy. It's not a "mod" but it is small with VV.

Vision eGo Spinner 400mAh Variable Volt Battery [02-A-1] - $20.00 : Healthcabin Electronic Cigarettes - Wholesale and Retail

As has already been said, the provari mini is the shortest regulated tube mod but they aren't cheap. One I haven't seen mentioned is the itaste SVD. Not the shortest available but like the telescoping zmax, you can use 18350, 18490, and 18650 batteries. I have one and I like it. You can find them reasonably priced as well. They aren't DNA powered but they are variable wattage.

I have an Itaste SVD and even with the 18350 battery it can be quite uncomfortable in my jeans pocket. BTW, I reall do like the itaste. :)

It's a mechanical mod. That means it has no voltage regulation and no built in protection from shorts.

Needed to add, it isn't hard to learn what you need to keep yourself safe. If you decide to go that route there are threads and blogs dedicated to teaching those things.
