Where Do Things Go Wrong, When They Do?

Hi,
This is a general question, related to what happened to my recent e-liquid experience.
Italian manufacturer Flavourart, but purchased not directly from them.

2 bottles of e-liquid.
1 very good with fabulous scent, the best one so far.
1 very bad. It's not just a matter of taste in this case. It was just plain bad. So bad that I am sure it wasn't meant to be the result, but something had gone wrong in the process.

Now question:
Where does it usually go bad when it does (in which part of the process)? Raw material selection, Mixing, storing, where?
Question is general, doesn't concern that particular example, because you can't know for sure what happened.

Well,not saying its anything pertaining to your case but:

-Dubious batch of nic juice (and I mean,I ran into some that were ''scented'')
-Lack of steeping (the case in most instances)
-Overflavoring
-Contamination
-Bad atomizer,gunked coil
-too high/low voltage/watts

This is only a guess, but what seems most likely to me:

1) Mixing-- some strong flavorants are used in very small quantities such as 0.1%. If too much was accidentally used, it could perhaps result in a bitter or harsh taste.

2) Contamination-- containers or equipment had detergent or solvent residue that combined with the eliquid ingredients.

Since this is a general question, I am not going to develop any theories about that bottle I bought. Just would like to see if anyone comes with another idea.

Retailer messed somethig with storage perhaps?
If kept in the light space instead of dark for some time? Did anyone have that type of experience (eg. you left it in inadequate place for a month or so and how did it impact the juice ).
