    could be a little of both, I have killed several of my attys doing a dry burn to clean them, simply let them burn too hot for too long. I suppose voltage could have done it, if they got way too hot too fast, that would do it too.

    it's easy to confuse a flooded atty or a dirty atty for a dead one, if you stop getting good vapor. If your atty is still getting warm, it is not dead, just needs to be rinsed out with alcohol and warm tap water, blow dry with the threaded end towards your mouth (like when removing the primer)...

    I have had one riva cone not stay on at all, too big to screw down, and a couple that were just really difficult to turn, poor quality control... if you got your kit from a reputable vendor, try to get it replaced, worse case, you can get a replacement for $3-4 on most sites.
