I just purchased a Li-Ion charger and two AW 18650 3.7V 2900mAh Li-ion batteries. I

put one of the batteries in my REO and the other in the charger and put it in my lunch bag so I could take it to work. When I got to work, I pulled the charger out and the bay where the battery was had melted. WTF? How did this happen without the charger being plugged in? The battery is fine. But what did I do wrong. I've carried my batteries in the charger before with no problems at all. Also I would like to note that this charger is brand new and never used. It came with a weird looking power cord that has two round prongs instead of flat. If you can help me understand that it would help. And the battery was installed properly into the charger. (+to+ -to-) ANY kind of help would be awesome. Thanks.
Below are pictures of the melted charger and the weird plug in.
