Creamy refers to a flavor note. It has nothing to do with mg of nicotine or anything else. For example: the juice that I use pretty much every day is a Peaches and Cream flavor. It tastes like exactly what it sounds like... peach with a bit of vanilla cream flavor. The nicotine is whatever I want it to be. Right now it is 7.5 mg/ml.... I've also had it at 15 mg/ml...
As for whether or not creamy is better... that's up to you to decide. It all depends on whether or not you like that flavor.
In the USA, creamy is considered a flavor as in buttery or cream tasting... it either tastes as if it has a cream flavor or it doesnt. I personally dont care for cream flavors due to the extended aftertaste....
As far as mg, I supposing you are wondering about how many of the cream flavor/extract is required to make it taste that way???

I see, because I've several friends with vape as well, one with a vape pen with the redbull flavor and the other wit a kanger subox mini with a blueberry and pineapple mix.
Somehow the former seems to be like vaping pepper into the throat(Extremely hard on the throat, it's even worse than red cig), while the other was much more soothing. Does the mod types affect your flavor in any way? I heard you have to change some parts as well, as it burns occasionally or something then the flavor would go bad. Thank you for your insight.

Yes, there are many things that can affect the flavor. The nicotine content, for one; high nic content can give a peppery flavor. Mod/atomizer type can definitely affect flavor. So can wattage settings. You also have to change out the wicks/coils from time to time as the wicks can burn and the coils can become gunked up... both will affect flavor. Basically every single part of the equation can make your vape taste great... or awful! LOL
