Likely a damaged o-ring. If you don't lubricate the carto and also use a tank accessory tool when moving the carto in or out of the end caps, you are probably stressing and possibly damaging the inner o-rings. These o-rings are the only things keeping juice from dripping out of the tank.

It might just be a bent carto, too. Easily solved by replacing that carto with a new one. Regardless, I do highly recommend getting a Slap yo Moma tank tool if you don't already have one.

I have the Phiniac tool in addition to the Big Daddy Vapors' tool, and I can say with total confidence that the SYM tool is hands down a better tool. I'm sure that there are folks that have and like the Phiniac, but I had issues with it and decided to try the SYM.

The Phiniac I had was difficult to remove from the carto because it fit tighter and because of its shape was difficult to grab with fingertips. It has cut slots to stick a fingernail into to assist removal, but its not enough.

The Slap yo Moma isn't as tight a fit on the carto, and is easier to grab with your fingertips because of its slim design. It also provides a larger space to drop your juice into when it is inside the tank. It is perfectly round with no sharp edges. The Phiniac does have edges at the point top, and theoretically could pinch on the o-ring if the angle isn't perfectly straight (just my observation, and not seen or proven). And isn't the idea of using a tank tool to protect the o-rings?

Another vendor sells a tank tool designed more off the SYM, perhaps Got Vapes?, don't know its price. Anyway, the SYM tool is also available in a delrin material that is cheaper than the stainless steel.

Be aware that this tool only assists with filling a tank with juice, it WILL NOT protect the o-rings when inserting a cartomizer into the top endcap during assembly. I see that it got mixed reviews even when used just as a tank refilling tool.
