Never is a strong word. Maybe never intentionally. Hopefully these guys are the exception. The following is a general statement and not specific to Halo. In fact, they actually used a very well established legit as heck lab to do their testing. They just need to keep it up because of what's below.

One thing I learned from the nutrition business is that unless you constantly test your end product and require your vendors to do the same and provide full disclodure on sourcing, one piece of paper saying "pass" means nothing. This usually requires a dedicated QC resource to constantly enforce it, visit supplier factories, etc. It costs a lot of money that I doubt most vendors are willing to spend. This is why brand/household name multimillion dollar supplement companies get nailed for failed protein claims, spiked products, etc. And this isn't some obscure brand. Many of these companies are victims of their suppliers, but not really since they could have constantly tested. This is stuff you can find at Kroger, walmart, etc. Think about that before believing a piece of paper, which many times is from a bogus lab. Just ask questions about the above points. If you get legalese, move on.

No, actually, never. They have never used diacetyl or AP from when they were founded. Making safer e-liquids has been their philosophy from the beginning...
They aren't like some other companies...

Like I said, they seem extremely legit. Perhaps even the most legit. In addition, using a lab that's been around for a couple of decades is huge. However, name any major food or personal use product company in history with no recalls, qc issues, etc. It happens. I like to allow my trusted vendors a tiny bit of room for error, so long as it's completely unintentional and not easily preventable. It's how they recover that's important. The rest of my statement was general.

Obviously there can be manufacturing errors, but If I could only trust one vendor to do everything right, it would be Halo...

Yeah. I really wish they had a 100% VG. PG makes me itchy and congested. That's the only thing that's kept me from ordering. Although I am also suspecting certain brands of vg do it too, thanks to feedback from HIC. Perhaps I will give theirs a shot just to see.

I think a lot of the PG sensitivities are due to quality of PG. I have heard of many vapers who can't tolerate high PG being able to vape Halo without issues...
