Did I get this DIY right?

So my aim is to get 5050 pgvg 12mg nic 40 ml.

I started with 5 ml 100mg pg based nic. I added 5 ml of VG to cut it to 50mg nic and 10ml.

Added 15ml vg and 15ml pg...total 40 ml

Seem right?

Also if I want to do 20% flavoring for 10 ml bottle theme just put 8ml of my base and 2 ml of flavoring? But since the flavoring is considered PG then my final mix isn't really 505pgvg, right? How to resolve this or does it not matter?

you really need to download the "juice me up" e-liquid calculator. if not that one there are others around too. after you use it a few times you will have a much better feel for mixing. what you are describing is a way too high concentration of nicotine so don't make that.

By my calculations, your base ends up as you said 50/50 12.5mg.
Adding your 20%PG base flavors would give you an end result of 60/40 at 10.0mg.

The 'juice me up' one downloaded fine for me but it wanted excel or something like that to work and I don't have excel.
