i came up with this idea / format to help narrow down issues and frequency

if yes the ip address:
repeatable across another browser:
yes if so which ones:
your current post count not including the one coming:
Error message if so what does it say:
Last login Time under your User name:
Current local time
and zone:
any diagnostic steps used on your end:
how often do you experience it:
are you only effected in certain areas:
what is areas are not effected:
Comments:
Fix: to be added by moderator in each ticket or case.
