The one's I've heard about were mods and involved unprotected batteries or improper battery use (recharging non-rechargeable type batteries and then using them.) If you buy a mod only use protected or safe chemistry batteries. Also look for vent holes and/or protection circuitry. This is covered in more detail by folks that know a lot more than I do in other places.

As far as a regular e-cig, like a 510, eGo, Kr808, etc? No, not that I have ever heard of. They have protection built into the batteries.

However, this HAS happened with mods. The reason is almost always user error. Typically, it is someone using unprotected batteries, and/or stacking batteries.

However, a mod used correctly is pretty safe. And for people using LiMN batteries in their mods, there has never been a reported failure. I am getting a mod soon, and plan to use LiMN batteries for this reason. There are many resources on the forum to read up on batteries, if you are going to use a mod.

Also make sure whatever mod you get has safety features, like venting holes and a locking/recessed switch.

So, for a stock e-cig kit - don't worry about it.

For a mod - make sure you use it correctly.
