how do you guys know when to change your batteries? I'm using a sony vtc4 18650

Measure it regularly until you get a feel for it.

I tested this the other night with 0 nic juice. From a fresh battery at 4.16v with a Patriot rda on a Dominus with a VTC5, .6 ohm dual coil and starting with saturated wick I'd get about 20 hits (until a dry hit) per .03v drop on the battery. Did this 5x in a row until I was vaped out. YMMV!

Vape a while then check your battery, eventually you will get a feel for it.
Carry extra batteries, since it's better to get back home with 2 half charged batteries, then an undercharged one.

Vapor production drops off big time. Seems like I pull mine around 3.6 Volts these days. You'll get used to it after a while. Pull it and measure it on the multimeter every now and then, you'll eventually just know.

I usually go one tank (Kayfun) per battery. That's changing it more often then I really need to. But like others suggested, measure it till you get a feel for when vapor production drops off. You will get the hang of it over time.
