Just ordered 4 15ml bottles. I read about a free bottle with $15 and up. Are they no longer offering this? Didn't see a place to choose flavor of sample...do they just throw it in?

You can add your request for selection of free sample in the comment box. No guarantee that they will be able to honor your request, but I suppose the less specific it is, the better.
They don't advertise it because they do not always do it. There is no policy, because there is no policy. Sometimes you get a sample, sometimes you don't.

Actually, I believe there is something under their FAQ section that says you'll get a free 15ml sample with every order over $xx. I placed my first order with them 2 days ago & received it today. My free sample was Blue Moo. While I really appreciate their full sized samples, I was a little disappointed at this choice because I had already ordered Utterly (something), Moo juice, & something else milkish so I would have rather tried something out of the ordinary rather than something similar to what I already ordered. From the way people have been describing Pooh, it sounds exactly like something I would love so I had my fingers crossed that it was going to be my free sample as well.
Editing this to add that I can't find mention on the FAQ page but I know I read it somewhere on their website before I placed my order. If you do some digging I'm sure you'll find it too.

You can also get free shipping on any order. Just put freeshipping in the coupon box.
I ordered from them too and received it today. Not only they didn't give me any sample, they sent the wrong size 
I ordered a 50ML and 2 bottles of flavors but got all 3 at 15ML each. Emailed their support and waiting now.

i have ordered 2 bottles at 231ml each twice and have not received crap for free samples from them. so I think its whatever they feel like that day.

I have been a loyal MBV customer for close to 2 years. I have never had any problems (surprises) with my credit card & have always gotten a free sample when I ordered over $15. They have excellent customer service and some of the fastest shipping in the industry (they are located in WA state & I live in GA, get my orders in days).
