I just got to the end of my Summer Peach sample bottle of Johnson Creek and Holy Cow! Can you say Peach? It tastes just like a real freakin peach. The rest of the bottle didn't have nearly this much flavor. It's almost as if all of the flavor settled to the bottom of the bottle. Are you supposed to shake the liquid before refilling?

Of course juice must be shaken (or stirred if you prefer).

PG/VG, nicotine and flavoring could become separated because of different densities.

One of the good examples: Italian dressing is based on olive oil and vinegar and after being on the shelf for a while you can easily notice that it consists of two liquids.
