You need the kit, a few spare atomizers (these are the little metal parts that attach to the battery and heat up to vaporize the juice) and juice. I would suggest getting small bottles of various flavors and maybe a couple of different nic strengths so you can determine what will work best for you. If you are a heavy smoker, then you might want nic strengths of 18mg -24mg, and get around 40-60ml of juice so you don't run out too quickly.

I got the ego from Liberty Flights. They have a kit for members of this form that's a killer price, it's a eGo which seems to work great. I tried to start with PG Juice and it killed my throat. PLease get PG and VG juice as well so you can try both. I used juicyvapor.com for my first juice order.....their website is very user friendly and makes it so so easy to find the juice you need.

One I've looked at is Pure. The prices look great, but haven't found any reviews in this forum. Has anyone here tried these?

Haven't tried that particular brand, but it looks like a re-branded RN-4081 model, and for $25:
Kit includes:
1 - 150mAH Pure Battery This would last about 1 1/2 hrs, give or take a bit, then recharge.
1 - Atomizer ULTRA My experience with 4801, don't last very long.
1 - USB Battery Charger
1 - Carrying Case
3 - Cartridge Refills For a 1 pad smoker, last a day.

Check out Electronic Cigarette - Starter Kits - Riva 510, they come with 2 ea -750 mah batteries, which is about 7.5 hrs on each before recharge. $35 but well spent, some sample bottles of juice and you have a kit that will perform well.

How long does the juice typically last?

That's going to vary from person to person. It all depends on how much you vape and how you vape. I use a Mod (Chameleon model) and drip with low resistance atomizers, so I typically go through 3-6ml per day. Some people might go through less.

There is absolutely no way for anyone to tell. I highly suspect that I vape more than I smoked because I tend to chain vape. The thing you have to remember is that vaping isn't like smoking and we don't absorb as much nicotine vaping as we did when smoking. In fact, we probably only take in 40%-60% of the nicotine in the liquid. The nicotine uptake is also slower and smoother with vaping, plus you aren't inhaling all of the other chemicals associated with cigarettes, so it doesn't feel the same.

There really isn't a way to compare cigarettes to e-cig cartridges or cartomizers, and there isn't a way to determine which nicotine level any one person should start using. There are some people who smoked more than a pack per day, but can't vape nic levels over 18mg. OTOH, there are people who smoked half a pack or less and have to vape 24mg to stay off the cigarettes.

Vaping is a lot of experimentation with both the juice and the hardware to get to that "sweet spot" that satisfies you.

+1 again!

Discovered the first day that 6mg (really low) nic juice can give me a headache/ringing in the ears if I chain-vape.... and I was a PAD smoker, Mrlb. 100's. (I just got new juice... I got 6mg and 0mg nic juice and mixed it to make apx 3mg nic juice. Now I can chain vape!)

If I had to guess, I'd say I'm vaping 2mls a day... but I direct drip with a 'drip tip', so I don't really keep up. :)

Try not to be too concerned about the upfront costs. If you start out with a good kit, you will not be throwing away money on the initial crap most of us bought at first. As WomanofHeart said, there is no way to know how much you will vape until you start. Buy a few different flavored juices (not just tobacco flavors) as you will find your sense of taste changes when you stop smoking.
