" Smokers invited to switch to electronic cigarettes who completely abstained from smoking showed steady progressive improvements in their exhaled breath measurements and symptoms scores. FeNo and eCO normalization is highly supportive of improved respiratory health outcomes and add to the notion that quitting from tobacco smoking can reverse harm in the lung."

Doctors that treat COPD have been noting for some time that there smoking
patients that switch to vaping almost always show improvement.
This presents a bit of a paradox as their never smoking patients do not
seem to have the improvement gains that are as dramatic. Medications
do help but, not all the time and not as near as much improvement than
switching from smoking to vaping in such a short period of time.

I am unaware of how smokers that completely quit make out. I do
know from past reading the prevailing opinion is once the damage from smoking occurs
it is more or less permanent and quitting may slow the progression but
not the ultimate outcome.

Looks like another study from Dr. Polosa's group:

"Among people with asthma and/or COPD (chronic obstructive lung disease, bronchitis and emphysema), improvement in symptoms was noted in two-thirds of smokers who switched to vaping. According to Dr. Polosa, taken together, these findings provide emerging evidence that e-cig/vapor product use can reverse harm from tobacco smoking."

Good observations and questions. I too would like to see smokers who quit by other means compared to vapers. The antibacterial nature of eliquid may be a 30 ton elephant in the ANTZ house. And note this is a European study. The FDA would never fund this. The truth would be too hurtful.

When you say completely quit, do you mean not only from combustibles but also quit from vaping?

I don't know about the ones who have also quit vaping, but I can tell you that as someone who has been "accidentally" smoke free for the last three years, I continue to experience improvements in breathing and fitness.

Blood gas CO2 levels dropped from 44% (I was a long-time two pack a day smoker) to 2% within three months - it might have been sooner, but I didn't have opportunity to do the testing until then. The ability to smell and taste flavour improved within a few weeks. Apparently, my snoring is less frequent and less sonorous (this "data" is from a tenant who lives on the floor above me).

I believe that once the body does not have to work so hard to counteract the effects of current smoking, repairs go on a-pace to systems that had been affected by combusted smoke.

I was referring to the what we are always told by the ANTZ and anti-smoking health
organizations and quit smoking types.
"How severe your COPD symptoms are depends on how damaged your lungs are. If you keep smoking, the damage will get worse faster than if you stop smoking.4 Among 15 million U.S. adults with COPD, 39% continue to smoke.5"

My experience: Having severe COPD I can tell you it really doesn't reverse-improves but does not reverse. The inflammation decreases after quitting which in turn stops most of the symptoms (thank god). I have underlying asthma which was never diagnosed before I quit because I could not go off any of my inhalers for an accurate PFT. After 2 months of vaping, I took the test without taking any inhalers and I had reversibility after albuterol was administered which was indicative of underlying asthma. All of my numbers improved. Although I have to take maintenance inhalers, 1-COPD, 1-Asthma, I have never felt better. When I smoked, I was on steroids 6 months out of the year for exacerbations and have had none (except when I was exposed to high VOC's from paint) since I started vaping. Even my pulmonologist was impressed. We don't need a scientific paper to tell us that we feel better. But I do applaud those pulmonologists who commit to the real science.
