Over time, nicotine will have a negative effect on the circulation to the kidneys, whether from smoking, vaping, patches, or any other means. There are no warning signs of this happening, it is just something that once it becomes apparent, it's too late to do anything about it, so, it is beneficial for any diabetic who is vaping to set a goal of stepping down to 0 nic eventually.

This is the exact advice the doctor gave. Due to the negative effect on circulation, she'll have to give up the nicotine.
