S;opes, the member who created "el ......," the world's smallest e-cig was also working on (finished maybe?) a model which worked with a tilt sensor. Naturally tilted upward, like one does while holding a cig, it is off. Tilted downward, as it typically is when taking a drag, it is switched on. He was working on putting in a cutoff timer so it wouldn't just continually burn if you happen to have it tilted a little more than intended. As far as holding it in your mouth, for most PVs, that's typically difficult because of the weight of the battery but the tilt sensor ...... may be a winning design for you because of its size.

I'm not sure if anyone has done any pressure-sensitive mods like some of the commercial models, but I know at least some of the cheaper ones use (or did in the past) a tiny microphone as a switch. The are problems with all automatics - specifically, they can turn on when you don't mean for them to. Also, they typically require you to draw air before they activate. However quickly that is, it's still an unnatural thing to draw and get nothing. That's why a lot of experienced vapers go with manuals. Modders prefer to build manuals because of the same reasons, plus a momentary pushbutton is cheaper and doesn't require cutoff circuitry.
That's not to say that you shouldn't use an automatic - just find the one that works best for you. On that note, have you tried any of the commercial autos? Maybe try a disposable and be sure it is what you want if you haven't. You may be surprised.
