LiteCigUSA has all of the hardware you're looking for. They also have juice, but I've never tried it. I get my juice from VaporBomb or Gourmet Vapor.
Heaven Gifts is not in the US, though. It's direct from China. Shipping will cost more and take longer than ordering from a US vendor. I've heard they're a great vendor, and they're on the ECF approved list, but I'd stick with ordering from the US until you get settled into vaping.

Litecigusa's adapters are a couple of dollars more than the adapters at goodprophets.com (which are 99 cents each plus shipping), so if you wanted several adapters, you'd save some money ordering them from goodprophets.
I get my juice from freedomsmokeusa.com, because I tried several suppliers and found I like FSUSA's juice the best.
So, you might have to buy from more than one supplier to get everything you'll need.
