I haven't use the adjustable yet but if you are comparing the voltage. On the go, I use the Nicostick at 3.7 it does a good job. At home I use a 5v mod and it is great.
I mainly drip.

There are a couple of good discussion comparing HV / variable voltage devices in the general discussion area. Think the main adjustable ones available are the Little Sister, Buzz, and Fist Pack.

The Little Sister is the most cost effective and adjusts with a iPod type wheel. Goes from 0 to 7 volts at the turn of the wheel.

The Buzz is less expensive then the FP and adjusts with a mini screwdriver I think. Both these require a voltage meter to determine the exact level at which you're vaping.

The Fist Pack is the most expensive and adjusts with a 4 level slider to either 4.3, 4.5, 4.75 or 5 volt. It's the only one that's USB chargable and can be used as a passthru. The FP is not available right now as Nuck is on vacation. He'll begin taking orders again in Sept. Understand he's looking into a modified version with more then 4 settings as well.

I have a Buzz (510 threaded) - its a recent purchase, I was using the 510 eGo before that and also my trusty PT, so Ill try to give you my impressions even though I havent had it long.

How long: Almost 2 weeks
What's failed: Nothing
Carts drip or carto: dripping
Fave liquids better: yes and also the same (ie not worse lol)
Yuk ones usuable: I have only tried one and I didnt hate it the first time - this time its more enjoyable.

I msut admit I tend to power through the batteries lol I can exhaust it in around 8 hours of constant (and by constant I mean ALL THE TIME lol) use. If Im not using it constantly then it lasts over 24 hours.
I am so far VERY happy with it - I love the ability to switch voltage for different juices (my fruity vapes prefer/taste better with a lower setting than my coffees for example) - the only (small drawback) is that I have no real idea which voltage Im on (other than 'low' or 'high') because theres no 'indicator' however this can be solved by a multimeter, which i plan on getting from a local store (a cheap one will do me fine). As its now the only device I use at home though thats not really a huge drawback :)

I have a 510 Fistpack SX and have had it for a little over 3 months. It is an awesome device and fits very comfortably in my small hand. I use it with both attys and cartos and the TH and vapor production are excellent. The ability to change voltages with a slider switch on the same device is also a godsend for an atty on its last legs. Kick the voltage up and your dying atty gets temporary new life. I also really like the usb option that acts as a charger and passthrough and love the fact that I don't have to use two batteries in series or paralell like the others. The FP uses one battery - an 18650, and it lasts me a good 12 hours. Another feature I love is the 4 light battery indicator on the bottom which lets you know when you need to recharge. This purchase was worth every penny IMHO.

The FP also has 4 different versions with the variables suited for your preference. This is from Nuck's site:

Models

Standard (SX) - The flagship model. The Fist Pack SX provides consistant power throughout the battery cycle. The settings have been tuned to provide the perfect experience at the touch of a switch. The SX model is designed for use with regular adapters and allows voltage selection ranging from approximately 4.3v to 5.0v.

High Voltage (HV) - The Fist Pack HV is identical to the SX with one exception. The voltage range has been boosted to between 4.5v and 5.2v for those that enjoy a heavier hit.

Low Resistance (LR) - A special version of the Fist Pack optimized for use with low resistance adapters. The power supplied to the connector is adjustable between approximately 7.9 and 10.6 watts. This version is also a great choice for those who want a consistant, fresh battery feel using standard adapters.

Economy (DX) - A simplified economical version. This unboosted, single voltage version provides extremely long battery life between charges. The Fist Pack DX combines all same great features of the Fist Pack with a high quality brand name battery. Simply the best unboosted performance you can buy.

How do you guys think that fist pack will hold up to High dust, vibration environments? I am a logger and run heavy equipment all day. Can it take a 3 foot fall ? That would be off the window sill of the shear to the floor... My 510 takes that trip 3-6 times a day. So far it has lasted. Lost the LED's in the tip of 2 no loss still work fine. The momentary switch he uses looks to be plastic. Think it can take neg degree temps ? We work up to -20's F. 30's I do my best to stay home. Id hate to hit it to watch it just snap off. Other then those fears its looks good.

Id like to take it and remake it into a juice box .. Can I get into it ?

Any joker AV's in here?

I carry my Fist Pack in a $10 cell belt clip I found at Target. Cushions it pretty well for falls, but the atty does stick out at the top. Heck, fits in there so well you could vape without removing it and just clip it back on your side when through.

Nuck has posted the diagrams and walk thru for making your own. (Think it's v2 rather then the v3 he's currently selling though.) I looked those over before purchasing and, though I'm not a modder, looks doubtful you could remake it into a juice box to me. There have been some looking into sticking a AFSv2 auto juice feeder on top, but could easily snap off in the aforementioned fall.

As for the environmental issues, I'd get into touch with Nuck either through his website or there contact info on the walkthru posted above. He would be the expert. Since he's based in Canada, he may even have an answer to the temperature question. Also, he's currently discussing a possible change from slider switch to recessed DIP switches that sounds very interesting and would give a larger range of voltage variables. :D

The Buzz is a great adjustable voltage PV with solid performance.
The Joker adjustable voltage is also very nice, but the button seems to be
a little harder to push, but very sleek in design.
The Little Sister is in no way little in performance. This PV pushes two 10440 batteries to their full potential. It is very small, about the size of a large Bic lighter. Of course,
due to it's smaller size, it doesn't hit quite as hard as the bigger mods, but adjusts nicely
for it's size.
Waiting on the Tekky multi-voltage PV. I have the 5v version and it is great. Solid performance with out of this world battery life.

You can buy a screw on voltage regulator now from the Buzz's site or even cheaper $22 over at NHaler.com drew has em over there and will be getting more this week that are adjustable instead of just 5v, this way you can screw this right on to your current mods and have you variable voltage.! Just a thought.!

The Joker AV is really nice. Very sleek in design with an adjustable screw like the Buzz has. It is a little lighter and can use the 3.0 Lifepo batteries as well as the 3.6/3.7 Lipo's. The button has a pronounced click when activated, but takes a little more effort than the Buzz's button.
Both the Buzz and the Joker AV are great......I lean towards the Buzz for durablility and all out performance.
