If you use a 3 cell battery holder and make a 3.7 mod can you disable and use the 3rd cell slot as an extra battery holder? I was thinking this would be a convenient solution to carry an extra battery but not sure (as I've never made a mod outside of my head) if there would be any issues with trying this. Thanks for any input.

You can do that, sure. Just put the positive contact at the top of the first slot as you would for a 2AA box.
I think it's much better to use the 2 batteries and a regulator though. I use this setup for a regulated 3.7V box and the 2 batteries actually last longer than 2 single cells do.

Rather than use the extra slot for a spare batt, why not use it for a working batt? Just wire them in parallel and you've got twice the run time without having to open and switch the batts halfway through.
