As a person who works in television I do see actors using PVs off camera all the time. Crew members too.

The issue, for USA TV at least, is a legal one. It is illegal to show anyone using tobacco in a TV show.
Vaping LOOKS a lot like smoking, so they avoid it to steer clear of legal woes.
Also, many Vaping devices are recognizable, that is also a legal issue when the shows go into syndication.
If you look closely most TV shows "Greek" out any brand names of anything that appears on camera unless that brand has paid the show to show it.
Of course, movies shown on TV are exempt from this.
As for why we don't see Vaping in movies? I think the phenomena of Vaping is not very old and they just haven't caught on to it yet. I think we will begin to see it more often.

There is a great TV series we watch me and my wife: it calld "Under the Dome"
Its based on a Stephen King book that I have ( well...I have most of is books).
It's a fresh new series I think from 2012, that happend in a nice country town like most of his
setups.
I won't put spoiler for others but in the last apisode one of the main line caricatures,
Pulled an analog Cigaret and the other caricature snack it from is hand ( she was the female ) and
He light it for her ( it was a nervouse kind of moment for them).
And after I saw your post, I wanted to say hay "Bernie look at that"

That is what I refer to:this is "OK" while it's not so actual any more.
So smokers that not in the market for e cig look and think to them self " same old same old".
And evry day with out tp put a change in concept is cutting our ex fellow "smokers" life shorter.

Read the whole thread, and I'm surprised no one is mentioning that big Tobacco has funded Hollywood for decades. One of the easiest ways to drum up a budget is to simply have an actor light up on camera and collect the money. It's still at work...

... I play a game when I watch movies, since I love to vape during good suspense: whenever an actress/actor lights up, I start vaping. On some movies I can't even keep up with the cast - too much nic even at 6mg. It really makes me notice how much the habit is subtly promoted _still_... My wife is no vaper, and when I remark, "wow, I couldn't keep up with those smokers in the movie," she simply says:

"What do you mean? I didn't notice that."

Meanwhile, I'm hurling from too much nic, trying to keep up with that Hollywood talent.
