Eric, Audio posted the same question over in the General e-liquid discussion area. A few of us offered tips to help out there, too. 
There was no hole in the cap. I drilled one with a 9/64 drill bit. Works fine now.

It just sounds like a defective insert. I've never heard of a hole-less insert before, but these things happen. It just didn't take the mold correctly.
Just be sure to always cap it well now because if the hole is too big, the liquid will leak from it if tipped upside down or in your pocket.

yeah....and don't remove the insert thinking it will be easeir to use an eyedropper or syringe...unless you want to dump a whole bottle of juice into your keyboard because you couldn't remember which bottles have the inserts and which ones you removed.....

I use to do that,but found if you just set it on top of a regular squeeze bottle,it will transfer just fine. It won't overflow,but it will fill it to the brim. Takes about 5 min to fill a 10ml bottle.I'm still trying to find a pipette that fits our bottles to offer as an option for whatever it actually costs us. Probably something around $0.75. Either that or offering syringes.
Got mine yesterday w/ the same problem, and yes, removing to use an eyedropper is a pain. Thanks for the posts, I'll drill holes in all 10 of the sampler flavors - they're delicious!

Widening the holes in the sampler bottles isn't really necessary. It only takes a few seconds holding them upside down to get the liquid out. The issue is really with the 10ml and 30ml bottles in that they take much longer for the liquid to come out on first use.
Sometimes I have found the 5ml bottles need a little tap or shake to get moving, but once they work, they work really well.

Yes, really like 'em! (now...ha!) The only tough thing is trying to decide which flavor I like from your sampler pack - they're all great!
