is mixing juice needed?

I'm pretty sure all the liquids in an juice mix do not have the same density. So, it's not a bad idea to give it a shake. However, there are so many variables here though that just saying it's the juice is probably not the answer. The level of battery charge, how wet the atty is (more is not always better), and how you happen to hit it probably have more effect.

Once the liquids are mixed, everything should be evenly distributed. Some flavorings tend to take longer (maybe days after first making eliquid) to evenly distribute / interact with the base liquid. But the nic won't 'settle' out.

I agree with the points that Mickey made, plus how much you vape within a certain time. With analogs when you finish one you're done; with vaping, you can 'vape too much' and eventually get increased TH, nic buzz, etc. If you are getting an increasing effect from the liquid, this could be the cause; not that the liquid is changing.

I find quite often that I feel different Throat hit, plus nic buzz with the same juice.
Is it possible that the nic is heavier and goes to the bottom of the bottle thus need to shake the bottle before using? or am I having problems with atties or my brain?

That is a great observation. But I think it's only true for some juices. TW Doppio Espresso must be shaken all the time. But it is so, so good. IMHO its good practice to shake up any juice.
