Regulated mods - battery life

So I'm extremely close to purchasing my first regulated mod. But I'm a little confused about one thing. It seems all of them have internal batteries and the option for an 18650 battery. I have multiple 18650 batteries but I'm a little confused about one thing. The charging cables that come with these mods. Are they charging an internal battery or charging the 18650? Is the 18650 the primary source of power for the mod?

Most either use a built-in battery, wich is typically charged via USB, or use a removable battery, which can be charged on a stand alone charger and sometimes via built-in USB charging.

For example, my iStick has a built in 18650 battery that must be charged via a USB charger. My VaporShark has a removable/replaceble battery that can be removed and charged, or charged by connecting USB power to the device like the iStick.
