I just picked up a Greendot Refill card at the local CVS on friday after work and they didn't ask me anything.
I've also had this card scanned and refilled at the register at RadioShack and Walmart and never had them ask for any info either.
The only time I had to provide any information was when I purchased the initial refillable card. I had to give them information and wait for the finished card with my name on it to come in the mail.
