The best information (In my own opinion) is in the DIY e-liquids forum here ( DIY e-liquid ) but here's a simple recipe. You should really go read up on that section before trying any of this as you can have dangerous problems when working with nicotine liquids. When I mix I have paper towels ready, a bowl of water for rinsing and clean up, and make sure there are no distractions. I measure with syringes and mix in 1 oz amber glass bottles with eye dropper tops. After mixing I label each bottle with the flavor and strength. The first time you mix, you should make small amounts and 0-nicotine, I usually cut this recipe by 10 and sample at 3ml. That way if you don't like the flavor you've not wasted a lot of material.

To make 30ml of Vanilla @ 5mg
3ml 50mg nicotine liquid eXtremevaping Xtreme Vaping
3ml flavor Mccormicks imitation clear vanilla (not the vanilla extracts, they will clog your atty)
16.8ml PG I get mine from a local drugstore.
7.2ml VG Available at most Walmarts - Humco brand - with the hand lotion

optional
a few drops of 151 Rum or PGA for throat hit.
a drop or two of pure stevia extract for sweetness

Mix today and let set until tomorrow. Shake after making and before using.

Again, you should go read the DIY e-liquid forum before trying this as there are a lot more options and a lot better instructions.

You can buy e-liquid with or without nic. Some people (self included) like to make their own juice and experiment with different flavor combinations. You can buy premixed ejuice with the nic strength and flavor that you want, from about a zillion vendors. The folks here will chime in with their favorites. If you want to make your own, take to ChuckB's great advice and check out the DIY forum.
