Who is eLiquid supplier in Brigham City UT?

I am running low on my favorite juice and can't remember where I bought it. Somehow the packaging survived a puppy attack but I can only read "...gham City UT 84302" on what's left of the label. Pretty sure it's "Brigham City." Anyone have any ideas? I have looked at tons of websites and their addresses - a few in Utah - but can't seem to find the right one. Does anyone know which ecig company this might be?

The puppy didn't ingest any eliquid...
