Yeah, it is very simple to use. Just input the amount of you Nic content, ratio of it, nic target, how much you want to make,what flavors you are going to add and percentages of them. Then at bottom you have area for the target final mix like 30PG/70VG, 50/50, 75/25, ect. ect. Then just hit calculate and on the right side it will show you how much of each ingredient to add. Most flavorings are PG based fyi. Unless you order otherwise.

Yes I got 5 flavors in today yellow cake, strawberry, blueberry, unicorn vomit, Bavarian cream. I looked at the calculator and it asked what %flavor did I want to use? Not sure about this.
It depends on the flavoring company, flavors & recipes.
It sounds like you got an order from Flavor West.
I've been using them almost exclusively for over 3 years.
The owner told me that all their flavors are designed to work at the same percentages.

I like it as strong as possible and found that these flavor %s work best for me:
12.5% in 70PG
16% in 50/50
18% in 60VG
20% in 65-70VG

Those are for Flavor West & Mt Baker Vapor only.
Different company's flavoring concentrations will be completely different.

There are recipes in some threads in the DIY forum. Read the stickies & @dannyv45 's blog

3mg per ml nicotine will not really require dilution. It is pretty low unless you are trying to get to 1mg or 2mg as a final strength. The flavoring you add is going to dilute the nicotine strength.

36mg per ml is a much more user friendly initial strength for nic solution. And 100mg per ml is a very common and economical and fairly safe if handle carefully.

Perhaps you already firmly understand this but it is unclear from your post.

@Mark Pregmon For flavors it is easier to start out low and add if you need more. The least I use in a mix is .5% on a strong flavor like Anise or Cinnamon ect. For Strawberry and Blueberry, I use 3% if it is a main flavor, Yellow cake I use around 1.5% which requires steep time. Bavarian Cream I use at 1-1.5%. I would suggest getting a sweetner or Cotton Candy to use as a sweetener. With what you have, not including Unicorn Vomit, I would try Blueberry @ 3%, Strawberry at 1.5% and Bavarian Cream @ 1%. Try it right away, then once after a day, then once at a week and then 2 weeks. Take notes as it will change over time. I mainly vape unflavored so lower percentages works really well for me. All my mixes are 30PG/70VG. Any questions, don't hesitate to ask.

Just so you are aware, 100mg/ml nicotine base is 10% not 100%. This is confusing for some when they first start DIY.

There are 1000 mg in a ml so a 100 mg/ml nicotine base is 100mg of nicotine and 900mg of PG and/or VG per ml.

Thank you so much for this information it's very helpful and you're right I got Flavor West...
IMHO the flavors that are the most spot on are Capella and TFA I have heard really good things about Flavorah as well but I don't have personal experience with it as of yet. Nicotine River has a great selection of flavors and nic, VG and PG as well. What strength juice are you wanting to make, starting with three mg it's got to be pretty low. I use 100 and mix down to 3 mg in my completed recipes.
