The only thing I remember causing a failed USB port in the last 600 or so systems we've built was someone stepping on the connector on a front mounted port. So far it has not caused any damage beyond a failed port. I've had 2 systems at one customer have that happen, and I warned them in advance about the position of the system. Had to cut the cable internally to one port only because it continually caused the OS to pop-up that there was something incorrectly inserted in the port.

Absolutely I've seen that many times. On our systems, we disco the front USB, add extra USB in the back, then bring it to the top of the machine with a velcro'd hub - or to the wall or work surface with a hub. Honestly - not a big fan of front USB. I've very nearly taken mine out a time or two swinging a chair around too fast or carelessly. Or puppy demanding my attention will hook it. The a cable gets pulled out to far, the chair rolls over it, and it gets caught-up in the wheels.
