I disagree with your suggestion of inference. The questions were "Were you not worried/concerned?" If the questions were regarding if Shazzy was stressed it shouldn't have included the word, "not" - as in, "Were you worried? Were you concerned?"

His wording made sense to me (American), but came off as possibly offensive to UK members. Yes, he was asking if she was stressed/anxious over her daughter, and maybe it contributed to gastro distress; not criticizing her parenting or her daughter.
